Adhesion Prevention in Laparoscopic Surgery
نویسنده
چکیده
Background: Adhesions have important consequences for patients, surgeons, and health services. Peritonealtissue injury can be prevented by using careful surgical techniques. A large number of antiadhesion products have been used experimentally and clinically to prevent postoperative adhesions. Methods: The current author reviewed the surgical literature published about epidemiology, pathogenesis, and various prevention strategies of adhesion formation. Results: Meticulous surgery is essential to reduce unnecessary morbidity and mortality rates from these untoward effects of surgery. Several preventive agents against postoperative peritoneal adhesions have been investigated. Bioresorbable membranes are site-specific antiadhesion products but may be more difficult to use laparoscopically. Liquids and gels have the advantage of more-widespread areas of action and increased ease of use, particularly during laparoscopic operations. Effective pharmacologic agents that can reduce release of proinflammatory cytokines or activate peritoneal fibrinolysis are under development. Their results are encouraging but most of them are contradictory. Conclusions: Many modalities are being studied to reduce this risk; despite initial promising results of different measures in postoperative adhesion prevention, none of them have become standard applications. With the current state of knowledge, preclinical or clinical studies are still necessary to evaluate the effectiveness of the several proposed prevention strategies for avoiding postoperative peritoneal adhesions. Laparoscopic technique in recent years has tremendously being used as a non invasive surgery to perform different operation in abdominal and thoracic cavities. This technique has been used from 1987 in different aspect of human surgery. In addition to diagnosis, biopsy, visualizing adhesion and neoplastic structure, recently cholecystectomy and ovariohysterectomy were also commonly being used a...

OBJECTIVE To provide a comprehensive review of recent epidemiologic data on the burden of adhesion-related complications and adhesion prevention. Second, we elaborate on economic considerations for the application of antiadhesion barriers. BACKGROUND Because the landmark SCAR studies elucidated the impact of adhesions on readmissions for long-term complications of abdominal surgery, adhesions...
متن کاملAdhesion formation after laparoscopic surgery: what do we know about the role of the peritoneal environment?
In spite of the approaches that have been proposed to reduce postoperative peritoneal adhesions, they remain a major clinical problem because of the associated intestinal obstruction, chronic pelvic pain, female infertility and difficulties at the time of reoperation. The pathogenesis of the process have been focused almost exclusively on the local events induced by the surgical trauma, and the...

Background and Objective: Post operative nausea and vomiting (PONV) are unpleasant feelings which frequently occur after laparoscopic cholecystectomy. The aim of this study was to compare the effectiveness of dexamethasone and propofol in prevention of PONV in patients undergoing laparoscopic cholecystectomy. Materials and Methods: 60 patients with ASA status 1-2, who had undergone laparoscopic...
متن کاملPostoperative adhesions and their prevention
The fact that adhesions can form following abdominal surgery has been known since the beginning of surgery. Yet during the early years of surgery, adhesion formation received little attention, the focus being on infection and survival. In the seventies clinical endocrinology developed explosively, driven by the introduction of oral contraceptives and by the introduction of radioimmunoassays—a t...
متن کاملChapter 23 Laparoscopic management and prevention of pelvic adhesions and postoperative pain
The consequences of pelvic surgery can include postoperative pain, adhesion formation, a recovery period, and postoperative fatigue. For the same intervention, laparoscopic surgery is considered superior to laparotomy since it is associated with less pain, a faster recovery, a shorter hospitalization period, and a more aesthetic scar. Laparoscopic surgery moreover is also believed to be less ad...
